Case
Management Programs
Through outreach and
advocacy, Planned Parenthood case management can eliminate barriers between
pregnant and parenting clients and services in the community. Case managers
provide home visits, ongoing education and assist clients with the
following:

Prenatal care, family
planning, WIC, etc.

Preventive
health education, immunizations
-
Healthy
birth outcomes
-
Parenting
education
-
Referral and advocacy
for health,
social and legal services
-
Follow-up support
There may be residency or
income requirements to participate in PPNNY’s voluntary case management
programs. We offer the Community Health Worker (CHW)
program and the Teen Age Services Act (TASA)
